Parents do not need to download the forms below unless original forms are misplaced or lost. All registered campers will be mailed a forms packet in early February or after registering. This packet contains a general information booklet (Bear Facts), 2 health forms (green and pink), release form (blue), activity selection form (orange), camper profile form (purple), a Yellow form for camp bank, transportation and early bunk items order. There will also be a pre-approval code for parents wishing to sign up for emails and online photos for the summer. The downloadable (pdf) forms below are for convenience should an original get misplaced or lost. All forms are due by April 1st. For campers registering after March 1st for the summer, we request that forms be returned as soon as possible.
